Dependent Varification
What You Need to Do
If you do not have family coverage, you do not have to do
anything. If you do have family coverage, you can complete the
verification online or on paper. If you choose online, log onto
https://Verify.secova.com/TN. Then follow the instructions. If you
choose paper, follow these steps:
1. READ the enclosed Definitions and Required Documents. It tells you who is eligible for coverage
and what you must send as proof of his or her eligibility.
2. GATHER the required documents for each dependent. Make copies. Write your name and
Identification Number in the top right corner of each copy. You can find your Identification Number
at the top of the enclosed Verification Form.
3. FILL IN the entire
Verification Form. Be sure to
sign it. 4. FAX the Verification Form and the required documents to Secova toll-free at 1-866-621-2160 by
September 4, 2009. If you do not have a fax machine, mail the Form and copies of all documents to
Secova. A postage-paid envelope is enclosed for your convenience. Be sure to keep a copy of the
Form for yourself.
After Secova looks at your documents, they will send you a letter to either confirm that your review is
complete or to tell you that they need more information.
Remember, if you do not give Secova proof of eligibility by September 4, 2009, your
dependents will be removed from coverage.

Tobacco Surcharge Information
IMPORTANT INFORMATION ABOUT THE TOBACCO USE SURCHARGE
On May 1, 2009, the state group insurance program launched the Tobacco Use Surcharge and Quitter's Refund. Since that time, nearly 10,000 plan members made the choice to quit using tobacco. This is an amazing response, and as you might imagine, we have received a large number of personal testimonials from people who have wanted to quit and now have been able to do so with the help of the tobacco quit aids, seminars and other supports.
On June 16, 2009, the General Assembly passed SB0205/HB0280, which delays the full implementation of the Tobacco Use Surcharge until 2011. We are therefore postponing the date of the surcharge by one year, until January 1, 2011.
Plans were to use surcharge revenue to pay for the new, expanded coverage of tobacco quit supports. With the postponement of the surcharge, suspending coverage of the quit aids was considered because of the new, unexpected expense. However, because so many members are now using the tobacco quit supports, coverage will continued at the $5 copay level until December 31, 2009.
If you use tobacco and are considering quitting, please talk to your doctor about the right approach for you. If you act now, you can use your insurance card to get the drugs, gums, patches or lozenges for $5 through the end of this year. If you every wanted to quit, you have every reason to do so now, and you have all the supports that you need.

Through December 31, 2009, state group insurance program participants will have access to additional resources if they choose to quit using tobacco. The benefits below will be available to all members regardless of their insurance plan, as part of the package to help tobacco users quit .
A prescription is required for ALL medications and supports, including over-the-counter therapies.
